Telephone lines: Monday to Thursday, 12:00 pm to 3:00 pm. Customer service staff are available to provide information about the application and document submission process.
Office hours (for in-person submission): If you have applied for an Assessment Report and you would like to submit your original documents in-person, you may schedule an appointment for in-person submission. Appointments for in-person document submission are only available to those with an 91Ï㽶ÊÓƵ File Number.

If you would like to authorize 91Ï㽶ÊÓƵ to discuss your file with someone else, please provide a signed, written letter with your application or documents. Please note that both the application form and document submission form must be signed by you, the applicant.
